Indemnity Forms

A completed and signed Indemnity Form is required for any sporting or cultural activity or school function off school grounds.  Such indemnity forms are handed out to the girls prior to the event.  Please ensure these are returned to the Teacher/Sports Coach concerned before the event.  Failure to do so will result in your daughter not being able to attend. 

We prefer you to return the hardcopy handed to you by your daughter, but sometimes circumstances make this difficult.  If this happens you may choose the appropriate indemnity on the right, fill it in to suit the occasion and email it to admin@chisijun.co.zw.  Please then inform the office that this has been done.
